Financial support to care for a sick child

Your child became ill, you missed work to care for them and prepare a benefit application.

RP 5052 — Application for child care benefit. This form is used to request support when you are absent from work to provide urgent and necessary care for a child or equivalent dependant because of illness or an accident. It records the applicant’s identity, the identity of the child or equivalent dependant and the other family details requested in the form, the periods of absence and the relevant declarations. It also covers adoption, civil sponsorship and foster families. You receive the blank original to complete and sign, with the required supporting documents. This application is not required when the situation is certified by a Certificado de Incapacidade Temporária issued by the competent Portuguese National Health Service (SNS) services.
