Recognition as an informal carer

You care daily for a dependent relative and want to apply for informal caregiver status.

CI 1 — Application for recognition of informal carer status. It is used for a first application or to amend an application concerning the care of a dependent person. The form records the carer, the person receiving care, the people they live with and the necessary declarations. You receive the blank original to complete, sign and attach to the specified supporting documents. Definitive recognition requires annex CI 12 and the applicable documents; medical declaration CI 17 is required in the situations set out in the instructions. The support allowance for the main informal carer must be requested separately using CI 2.
